Radiopharmaceuticals
Rb-82 requires a generator, daily QC, and is best suited for high-volume practices. F-18 Flurpiridaz allows for unit doses and treadmill stress testing and can be a benefit to small practices.
Cardiac PET/CT
A cardiac PET program requires attention to several details such as generator logistics and availability, qualified and certified CT technologists who can do the calcium score you were promised, and list-mode data that requires increased storage and processing capabilities.

Equipment choices
Rb-82 requires a generator, daily QC, and is best suited for high-volume practices. F-18 Flurpiridaz allows for unit doses and treadmill stress testing and can be a benefit to small practices.
PET/CT scanners allow for quantitative MBF/CFR and definitive microvascular and multivessel detection.
CT doesn’t just allow for attenuation correction, but can provide diagnostic CT imaging such as calcium scoring as an adjunct to the cardiac PET study. Diagnostic CT studies must be performed by qualified individuals.
Proper setup is critical and needs to be appropriate for the conditions of the radioactive materials license. Customized software and hardware can eliminate the need for paperwork, improving efficiency and regulatory compliance.
PET/CT cardiac studies require a greater amount of data in comparison to standard PET/CT studies. Without optimization, a study might be completed quickly, but the processing of the results from poor planning can lead to unnecessary delays.
Accreditation submission assistance can free up technologists and other staff to perform other tasks.
Protocol optimization
| Tracer | What the day actually hinges on | What we tune |
|---|---|---|
| Rb-82 | Generator elution, ~76-second half-life, rest–stress in about half an hour, pharmacologic stress | Elution-to-injection window, list-mode framing for flow, CTAC order, motion on the dynamic frames, dose versus peak count rate |
| F-18 FDG viability | Glucose handling, rest perfusion companion (PET or SPECT), slices the reader will compare | Prep, timing against perfusion, display pairing, labeling for a viability submission |
| F-18 flurpiridaz | Unit dose, longer half-life, exercise-capable perfusion without a generator | Rest–stress separation, residual activity, whether flow quantification is part of the evaluation for that site |
Protocol work follows ASNC PET MPI practice and the camera’s validated reconstruction — not a generic timing card. Pharmacologic stress agents and exercise labs stay under the site’s existing medical direction.
